You are viewing this form because you have elected to engage in asynchronous telemedicine visits with a licensed healthcare practitioner through the Medura platform. Your healthcare services may be provided by a licensed physician, nurse practitioner, physician assistant, or other qualified healthcare professional authorized to practice in your state. By using this site, YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTAND THE TERMS PRESENTED BELOW, UNDERSTAND THE RISKS AND BENEFITS OF ASYNCHRONOUS TELEMEDICINE, AND HEREBY GIVE YOUR INFORMED CONSENT TO PARTICIPATE IN ASYNCHRONOUS TELEMEDICINE UNDER THESE TERMS.
What is Telemedicine?
Telemedicine is the delivery of healthcare services, including examination, consultation, diagnosis, treatment, and follow-up care, through electronic communication technologies when you (the patient) are located in a different location than your healthcare practitioner.
Telemedicine may allow you to receive treatment from a healthcare practitioner without traveling to another location or scheduling an in-person appointment and may improve access to healthcare services.
What is Asynchronous Telemedicine?
Asynchronous telemedicine is one method of delivering telemedicine services. Asynchronous communication is often referred to as "store-and-forward" communication because information is submitted, collected, reviewed, and responded to at different times.
Examples of asynchronous communication include: Secure patient questionnaires, Text-based communications, Secure messaging, Submission of photographs, documents, laboratory results, or other medical information, and Other electronic communications that do not occur in real time.
Asynchronous telemedicine differs from synchronous telemedicine, which generally involves live audio or video interactions between patients and healthcare practitioners.
Healthcare practitioners may utilize asynchronous telemedicine when live communication or face-to-face interaction is not necessary or appropriate.
The Medura platform currently supports asynchronous telemedicine through: Text-based healthcare practitioner-patient interactions utilizing SMS and MMS communications, and Secure information collection through asynchronous patient questionnaires and other store-and-forward technologies.
What Are the Possible Risks of Asynchronous Telemedicine?
As with any healthcare service, there are potential risks associated with the use of asynchronous telemedicine. Potential risks include, without limitation:
• Delays in medical evaluation, consultation, diagnosis, or treatment due to equipment failures, technical difficulties, poor data quality, telecommunications interruptions, Internet outages, or other service disruptions.
• Security failures that could result in unauthorized access to personal health information despite reasonable safeguards.
• Incomplete, inaccurate, or omitted information provided by the patient may result in adverse treatment outcomes, delayed diagnosis, medication interactions, allergic reactions, or other negative outcomes.
• The healthcare practitioner may not have access to your complete medical history, medical records, laboratory results, imaging studies, or other information that may be relevant to your care.
• The healthcare practitioner may determine that asynchronous telemedicine is not appropriate for your condition and may recommend an in-person evaluation, urgent care visit, emergency care, diagnostic testing, specialist consultation, or referral to another healthcare provider.
• Limitations associated with the absence of an in-person physical examination may affect the healthcare practitioner's ability to diagnose, assess, or treat certain conditions.
Asynchronous telemedicine services are NOT emergency services. Messages, questionnaires, uploaded information, and other communications submitted through the Medura platform are not continuously monitored and should not be used for emergency medical situations. IF YOU BELIEVE YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY OR SEEK EMERGENCY MEDICAL CARE.
THE CARE YOU RECEIVE WILL BE AT THE SOLE PROFESSIONAL JUDGMENT OF THE HEALTHCARE PRACTITIONER PROVIDING YOUR CARE. NO DIAGNOSIS, TREATMENT PLAN, PRESCRIPTION, OR SPECIFIC OUTCOME CAN BE GUARANTEED. THE HEALTHCARE PRACTITIONER WILL DETERMINE WHETHER YOUR CONDITION IS APPROPRIATE FOR TREATMENT THROUGH ASYNCHRONOUS TELEMEDICINE.
Your Rights and Acknowledgements
By participating in asynchronous telemedicine services, you acknowledge and understand the following:
• Your healthcare information will be handled in accordance with applicable federal and state privacy laws and professional standards.
• Identifiable medical information will not be disclosed to researchers or unrelated third parties without your authorization except as permitted or required by applicable law.
• Asynchronous telemedicine may involve the electronic transmission of your healthcare information to healthcare practitioners, pharmacies, laboratories, service providers, or affiliated healthcare entities that participate in your care.
• You may reasonably expect to benefit from asynchronous telemedicine; however, no specific results, diagnoses, treatment outcomes, or prescriptions can be guaranteed.
• Information submitted through the Medura platform, including questionnaires, messages, photographs, videos, documents, and healthcare practitioner documentation, may become part of your medical record.
• Communications from your healthcare practitioner may include the practitioner's name, credentials, and other identifying information, and such communications may become part of your medical record.
• You may withdraw your consent to participate in telemedicine at any time by contacting Medura. Withdrawal of consent will not affect information previously collected, disclosed, or documented as permitted by applicable law.
• Your healthcare information may be shared in accordance with Medura's Privacy Policy and applicable federal and state laws.
• Your healthcare information may be disclosed when permitted or required by law, including: Compliance with a valid court order or legal process, Reporting suspected abuse, neglect, or domestic violence, Preventing or reducing a serious threat to health or safety, or Other circumstances authorized by applicable law.
Call, Email, and Text Messaging Consent
You expressly consent to receive communications from Medura and participating healthcare practitioners by telephone, email, SMS, MMS, and other electronic means regarding: Healthcare services, Appointments, Treatment recommendations, Prescriptions, Order updates, Account activity, Administrative matters, and Other communications related to your use of the Medura platform.
Some communications may be sent using automated technology, including automated dialing systems or prerecorded messages, where permitted by law.

Consent to receive communications is not a condition of purchasing any goods or services.
You may withdraw this consent at any time by contacting support@medurahealth.com or following applicable opt-out instructions.
Duration of Consent
This Asynchronous Telemedicine Informed Consent shall remain effective unless withdrawn by you or otherwise terminated in accordance with applicable law.

By using our site, you acknowledge that you have had the opportunity to review this consent, ask questions regarding telemedicine services, and voluntarily consent to receive healthcare services through asynchronous telemedicine when determined appropriate by your healthcare practitioner.
